At the same time I declare that I have read the information about the administrator and the rights of the person whose personal data are processed by the Administrator, placed below and in point 8 of the Regulations. *

  1. The administrator of Users’ personal data is Szpital Miejski w Zabrzu Sp. z o.o..
  2. Your personal data will be processed by the Controller on the basis of your consent only for the sole purpose of ensuring the consideration of the offer.
  3. With respect to the data of Suppliers, Business Partners and the Administrator's Clients registered on the Procurement Platform https://szpitalzabrze.logintrade.net Logintrade S.A. is also a controller, with its registered office in Wrocław (ul. Braniborska 2/10 53-680 Wrocław), KRS: 0000438056, e-mail address: kontakt@logintrade.pl entrusting data processing LT Serwis Sp. z o.o. with its registered seat in Warsaw (ul. Polna 48 /11 00-644 Warsaw), KRS: 0000311815, e-mail: kontakt@logintrade.pl.
  4. Your data will be processed by Logintrade S.A. And LT Serwis sp.z o.o on the basis of your consent for the sole purpose of ensuring the fulfillment of the tasks of the Procurement Platform.
  5. Your personal data is processed for the period necessary for the performance of the service.
  6. You have the right to access the content of your personal data and to rectify, erase or limit their processing and rectification, and the right to object to the processing and to transfer the data.
  7. You have the right to withdraw your consent to the processing of your data at any time. The exercise of the right of withdrawal shall not affect the processing, which was carried out on the basis of the consent prior to the withdrawal.

  12. Please be advised that the Controller, Logintrade S.A. and LT Serwis sp. z o.o. do not envisage the processed data to be made available to any recipients of this data.
  13. The administrator does not allow the use of the services listed in these Rules anonymously or under a pseudonym.
  14. Your personal data will not be processed automatically and will not be profiled.
